How to proceed the paper:
1. UUDECODE the lines between BODY and ENDBODY. You will
receive combfey0.zip file.
2. Unzip the banwave1.zip file. You will receive 5 files:
   combfey0.tex
   combfey0.bbl
   myart98.sty
   mydef.tex
3. Run LaTeX2e twice on combfey0.tex file.
