[Maxima] Maxima by Example Ch. 1 and 11 updates

Maxima by Example, ch. 1, Intro. to Maxima,
and ch. 2, Fast Fourier Transform Tools, have been
updated to Maxima v. 5.19.0.

Ch. 1 has more examples of user defined maxima functions,
including recursive and indexed functions.

Ch. 11 has been greatly simplified (as well as the assoc.
software package qfft.mac) because of the new version
of fft.lisp which allows list-based use.

Complete latex source code files have also been 
posted for these two chapters as a convenience 
for instructors who wish to incorporate parts
of these chapters in their own class notes and
handouts.
