Among the groups arising as orthogonal groups of integer quadratic forms, which are maximal under inclusion? And what are the simplest quadratic forms from which each such group arises? We explain the answers for indefinite forms with rank at least 3. The proofs involve a novel non-positively curved simplicial complex, and a detailed analysis of certain 2-adic integral orthogonal groups.
